Maple Street Clinic has been providing family medical care in Forest Grove since 1964. We are locally owned and operated by caring physicians committed to providing quality medical care. Our providers include:

Douglas Holmes, D.O.

Holmes.jpgA graduate of Willamette University and The College of Osteopathic Medicine and Surgery in Iowa, Dr. Holmes completed his Residency at Eastmoreland Hospital in Portland, Oregon. He joined Maple Street Clinic in 1978.

As a physician and owner of Maple Street Clinic, Dr. Holmes has been the trusted doctor of multiple generations of families. Understanding that his patients are all in different stages of life, Dr. Holmes is proud to practice a “whole person” approach to medicine and is committed treating the underlying cause of a condition.

Outside of practicing medicine, Dr. Holmes is very involved in his church community and also sponsors two international children. He has been married to the love of his life, Mary, for 37 years. Together they enjoy golfing, camping, sports of all kinds and spending time with their grand-kids.

Roger Reynolds, D.O.

Reynolds.jpgA graduate of Southwestern Oklahoma State University and Oklahoma State University College of Osteopathic Medicine and Surgery, Dr. Reynolds completed his Residency in Wichita, Kansas. He has been practicing medicine for over 30 years and has been with Maple Street Clinic since 1993.

As a physician and owner of Maple Street Clinic, Dr. Reynolds believes the doctor-patient relationship contributes significantly to the quality of care delivered and has built his practice on this belief. He very much enjoys working in a smaller community where he may get to know his patients on a more personal basis.

Outside of practicing medicine Dr. Reynolds enjoys hiking and is thankful to live in Oregon where there are many places to explore. He also likes to travel.

Conley Lynch, M.D.

Dr-Lynch-30sep15.jpgA graduate of Carroll College in Montana and the University Of Washington School Of Medicine in Seattle, Washington, Dr. Lynch completed his Family Practice Residency at the University of North Dakota in Fargo, North Dakota. He joined Maple Street Clinic in 1996.

As a physician and owner of Maple Street Clinic, Dr. Lynch has a very loyal patient base. His professional, yet approachable, demeanor puts his patients at ease. He works directly with his patients to understand their issues so that they may take part in the decision making process about their health.

Outside of practicing medicine, Dr. Lynch has a talent for writing. He also enjoys movies and spending time with his three teen-aged children.

Joy Gephart, P.A.–C

Gephart.jpgAfter earning a Bachelor of Science degree in Exercise and Sport Science from Brigham Young University in Hawaii and a Master of Science degree from Pacific University's Physician Assistant Studies program, Joy joined Maple Street Clinic in 2007.

She is proud to improve quality of life for her patients and provides clear, honest communication. Joy focuses on comprehensive care for the entire family, with an interest in women’s, young adult, and teen health.

Outside of practicing medicine, Joy is an active member of her church community. She enjoys being outdoors and making memories with her daughters and husband.

Melissa Walsh, P.A.–C

After graduating Cum Laud with a B.S. in Medical Technology from University of the Sciences in Philadelphia, Melissa worked as a Clinical Laboratory Scientist for several years taking her career in a different direction. Melissa earned an M.S in Physician Assistant Studies from Pacific University in 2014. She joined Maple Street Clinic at the start of 2017.

Melissa stresses education and self-advocacy to her patients and believes that good medical care begins with open communication. She also has a passion for serving the under-served and for giving back to the community.  In addition to her career at Maple Street Clinic, she volunteers at the Hillsboro Free Clinic, where she, not only helps those in need, but also helps the next generation of PA’s by precepting for PA Students.

Outside of practicing medicine, Melissa is an avid Crossfit participant.  Melissa has a love for travel and photography. She very much enjoys immersing herself in new places and with different cultures. She has lived in many places around the world but is happy to now call the Pacific NW her home.
